NDIS Supported Accommodation Service Coordinator (SIL)
About the role
We’re seeking a passionate and experienced candidate for the role of NDIS Supported Accommodation Service Coordinator (SIL). You will support the objectives of the Myhorizon Accommodation Service (MAS) division by working closely with new and existing participants, their nominees, and key personnel.
Your role will focus on assisting participants with disabilities to live independently in their own homes and to promote positive outcomes. Additionally, you will lead a team of dedicated support workers across various Supported Independent Living (SIL) homes.
The is a full-time contract position, with a 12-month duration.

Required Experience, Skills and Qualifications

Minimum diploma qualification in Individual Support, Social Sciences, Management, or significant demonstrated experience and skills in human services support.

Knowledge of the NDIS, NDIS Practice Standards (SIL) and Code of Conduct within the Quality and Safeguard Commission frameworks. Including an understanding of Positive Behaviour Support Plans and Restrictive Practices.

Intermediate to advanced IT Skills, including Office 365 applications.

Valid 100 points of ID (Passport, Birth Certificate and valid Open QLD Driver’s Licence).

Valid Provide First Aid Certificate, including CPR or ability to obtain before commencement.

NDIS Disability Worker Screening Clearance.

Working with Children’s Check – Queensland (Blue Card).

Demonstrated knowledge of effective service planning, implementation, and evaluation.

Advanced interpersonal skills.

Ability to work as part of a team and autonomously.

Experience in building and leading successful teams.

Ability to demonstrate leadership in staff recruitment, training, mentoring and the staff review process.

Exceptional oral and written communication skills with experience in supporting non-verbal communication.

Proficient in prioritising and coordinating tasks amidst competing demands to contribute to successful participant and organisational outcomes.

Demonstrable knowledge and expertise in collaborating with people with disabilities and their nominees.

Commitment to providing professional, high-quality service delivery.

Knowledge of active support and person-centred support to assist people with disabilities to participate in their lives.

Knowledge of conflict resolution processes and facilitation of successful outcomes.

Supported Accommodation Professionals
About the role
Myhorizon are seeking passionate and experienced candidates to fill multiple roles as Supported Accommodation Professionals. This role will be responsible for supporting participants in a 24/7 Supported Independent Living (SIL) setting within NDIS Practice Standards. This position requires you to be available to work during the day, in the evenings, sleepovers, on weekends and public holidays.
Role responsibilities

Empower participants to achieve their goals by enhancing abilities and capabilities

Methodically monitor goal progress and evaluate support effectiveness, ensuring accurate documentation
-
Deliver direct support, guidance, and supervision aligned with participants’ NDIS plans
-
Facilitate the development of specific goals outlined in participants’ plans, offering continuous support
-
Coordinate appointments, follow-up tasks, and plans to meet participants’ needs effectively
-
Provide necessary physical assistance and personalised care as specified
-
Managing complex needs or disability-related behaviour when required, following Care Plans, Risk Assessments or Positive Behaviour Support Plans (PBSP)
Required Experience, Skills and Qualifications
-
Equivalent Disability related qualification such as Certificate III in Individual Support (Disability) and/or willing to obtain CHC33021 Certificate III in Individual Support (Disability) within your first 12 months of commencing employment (with our RTO provider, Horizon Specialised Training, RTO Code: 30008)

Demonstrated experience and skills in community services delivery are highly desirable

Demonstrated knowledge and experience in delivering “person-centred” support within a SIL or Lifestyle environment

Ability to work in a team or independently

Demonstrated ability to plan, prioritise and organise your workload in a way that contributes to high levels of successful participant outcomes

High level of problem-solving skills

Intermediate IT skills

Current NDIS Worker Screening Check or eligible to obtain prior to commencement (no card, no start rules apply)

Working with Children’s Check (Blue Card) or willing to obtain prior to commencement

Current QLD Driver’s Licence and willingness to use own vehicle with reimbursement for kilometres travelled

Current First Aid Certificate including CPR and/or willing to obtain prior to commencement

Australian Citizenship or Permanent Residency is required

COVID-19 Vaccination or medical exemption highly recommended
