Supported Accommodation Professional (NDIS Supported Independent Living)
The Role
Our stand-out Supported Accommodation Professionals (SAP) are responsible for supporting participants (adults who have disabilities) with their goal of living in their own home.
To join our vibrant team you will need to be prepared to actively take responsibility for the provision of direct support for participants to develop, monitor and achieve their personal goals both within their own home and in their local community in a 24/7 supported accommodation setting.
You will be responsible for:
- Strengthening the abilities of participants and building their capacity to achieve their daily living goals.
- Monitoring the progress of participant goals and recording the effectiveness of their supports.
- Providing direct support, supervision and guidance of participants in daily living tasks including domestic and household duties.
- Supporting participants to enhance and develop specific goals as identified in their NDIS plan.
- Assisting with appointments, follow up activities and plans.
- Providing physical assistance and personal care as required.
- Managing disability-related behaviour or complex needs in the home.
- Maintaining participant records and reports.
Required Experience, Skills and Qualifications
- Significant demonstrated experience and hold or willing to obtain Certificate III in Community Services or Individual Support (Disability)
- Demonstrated knowledge and experience in delivering “person centred” supports
- Ability to work harmoniously in a team or independently
- Demonstrated ability to plan, prioritise and organise your workload in a way that contributes to high levels of successful participant outcomes
- High level of problem solving skills
- 100 points of ID including current passport/birth certificate and current QLD driver’s licence
- Certificate of Completion – NDIS Worker Orientation Module Certificate
- Certificate of Completion – New Worker NDIS Induction Module Certificate
- Certificate of Completion – Dept. of Health, COVID-19 Infection Control Training and/or DSC – Supporting People to Stay Infection Free
- Current Apply First Aid Certificate including current CPR
- NDIS Worker Screening, current Paid Blue Card and or current Yellow Card – Positive Exemption Notice
- Computer Skills – MS Office (Word, Outlook, Internet etc
Disability Support Professional (NDIS Community Participation)
The Role
Our stand-out Disability Support Professionals (DSP) are responsible for supporting participants with their goals regarding community participation and independence.
To join our vibrant team you will need to be prepared to actively take responsibility for the provision of direct support for participants to develop, monitor and achieve their personal goals either within their own home or in their local community, we have community based Hubs that can be utilised to work towards specific goals.
All positions offered are permanent- part time positions.
Please note the majority of the support offered/shifts available are Monday to Friday between the hours of 8am to 3:30pm.
Myhorizon provides supports across: Logan, Gold Coast, Wynnum and Redlands
You will be responsible for:

Required Experience, Skills and Qualifications
- Qualifications at Certificate III level or above in; Individual Support, Community Services, or Social Sciences, and/or significant demonstrated experience and skills in community services delivery.
- Demonstrated knowledge and experience in delivering “person cantered” supports.
- Ability to work in a team or independently
- Demonstrated ability to plan, prioritise and organise your workload in a way that contributes to high levels of successful participant outcomes.
- High level of problem solving skills.
- Intermediate IT skills.
- Well-developed oral and written communication skills.
- Hold a Blue Card/Yellow Card or NDIS screening card.
- Current QLD Driver’s License
- Current Apply First Aid Certificate including CPR
Employment Coordinator
The role
Myhorizon is offering an exciting opportunity for an experienced self-motivated and passionate Employment Coordinator to join our Employment & Training team based at Stones Corner. This is a permanent full-time position.
Working with a friendly team, you will be responsible for coordinating training and placement through case management of a diverse group of individuals, working closely with Employers, the local community and industry groups to secure sustainable work opportunities for our Participants.
The candidate
To be successful in this role you need to demonstrate the following:
- Proven ability to engage and motivate participants by facilitating work experience, work trials, and per-vocational training with the aim to achieve sustainable employment.
- Strong organisation skills and attention to detail, maintain quality record management systems and compliance with DES framework and guidelines.
- Ability to work in a team environment to uphold Myhorizon mission and values, exercise autonomy to case manage and coordinate work activities.
- Build rapport, and influence both internal and external stakeholders to provide job opportunities for people with a disability.
- Sound understanding of Microsoft Suite programs, and ability to acquire and retain information from database programs and Government Employment System
