Supported Accommodation Professionals
About the role
Myhorizon are seeking passionate and experienced candidates to fill multiple roles as Supported Accommodation Professionals. This role will be responsible for supporting participants in a 24/7 Supported Independent Living (SIL) setting within NDIS Practice Standards. This position requires you to be available to work during the day, in the evenings, sleepovers, on weekends and public holidays.
Role responsibilities

Empower participants to achieve their goals by enhancing their abilities and capacities

Methodically monitor goal progress and evaluate support effectiveness, ensuring accurate documentation
-
Deliver direct support, guidance, and supervision aligned with participants’ NDIS plans
-
Facilitate the development of specific goals outlined in participants’ plans, offering continuous support
-
Coordinate appointments, follow-up tasks, and plans to meet participants’ needs effectively
-
Provide necessary physical assistance and personalised care as specified
-
Managing complex needs or disability-related behaviour when required, following Care Plans, Risk Assessments or Positive Behaviour Support Plans (PBSP)
Essential requirements
-
Minimum Certificate 3 in Individual Support, community services, or social sciences, or the ability to obtain Certificate 3 within the first six months of employment. Significant demonstrated experience and skills in community services delivery are also highly desirable

NDIS Worker Screening Card (Yellow Card) or willing to obtain before employment

Working with Children’s Check (Blue Card)
-
Current QLD Driver’s Licence and willingness to use own vehicle (with reimbursement for kilometres travelled)

Current First Aid & CPR Certificates

Right to work in Australia
-
100 points of valid ID

Covid vaccinations required

Disability Support Professionals
About the Role
Myhorizon are seeking passionate Disability Support Professionals who are looking to support and empower individuals to achieve their goals through meaningful community engagement and personal development, within NDIS guidelines. The role will also promote and develop independent skills, manage challenging behaviours, assist with personal care, and provide support to access our community hubs or community programs.
The position requires flexibility to work Monday and Friday including some weekend shifts across various locations such as Redland City, Brisbane City, Logan City and the Gold Coast.
Essential Requirements

Demonstrated knowledge and experience in delivering “person-centered” support
-
Certificate 3 or 4 in Disability or Individual Support is highly regarded. However, candidates willing to work towards Certificate III CHC33021 Individual Support (Disability) within 12 months of employment will also be considered

Willingness to work several days a week, including some weekend shifts

NDIS Worker Screening Card (Yellow Card) or willing to obtain before employment

Working with Children’s Check (Blue Card)

NDIS Worker Orientation Module

Current First Aid & CPR Certificates

Manual Handling certificate or willing to obtain

Two current professional references (within the last 2 years) from a supervisor (essential)

Medication certificate or willing to obtain

About You

Ability to work in a team and also independently.

Genuine desire and commitment to provide the best quality support for our participants.

Demonstrated experience and skills within the disability or community service industry are highly desirable (min of 1+ years experience).

Demonstrated experience working and supporting people with complex disabilities and challenging behaviours (desirable but not essential).

Excellent written and verbal communication skills.

High level of problem-solving skills.

Intermediate IT skills.

Roadworthy and fully insured reliable vehicle and a willingness to use your own vehicle to transport participants.
